What is Direct Primary Care?

Direct Primary Care (DPC) is an innovative alternative payment model improving access to effective health care with a simple, flat, affordable membership fee. No fee-for-service payments. No third-party billing. The defining aspect of DPC is a long-lasting and trusting relationship between a patient and their primary care provider. Patients have extraordinary access to a physician of their choice for a low monthly fee. DPC is embraced by health policymakers on the left and right as it creates happy patients and happy doctors all over the country! 

 

       Better Health Patients are able to achieve superior health outcomes with Direct Primary Care's unique delivery model. DPC offices provide better access to physicians, empower an authentic therapeutic relationship, and comprehensive patient care.

     

       Lower Costs - It is affordable and transparent costs. Patients pay for their care directly to the physician. No third parties or Fee for Service billing ("FFS") to inflate costs. Most DPC memberships cost less than the average cell phone bill.

 

       Enhanced Patient Experience Patients receive unrestrictive access to their healthcare provider, report little to no wait time, and longer appointments (in person, virtually, or phone). In turn, creating a real therapeutic relationship between patient and provider.

 

What kind of practice is Ace Integrative Health?
Ace Integrative Health is a patient-focused primary care office specializing in anti-aging, personalized, holistic and integrative care.  We are Direct Primary Care Practice meaning office services are charged by membership fees rather than contracting with a health insurance plan.

 

Is Direct Primary Care and Concierge Medicine the same?
No.  Concierge Medicine may charge insurance, co-pays, and co-insurance along concierge fees to pay for services.  Direct Primary Care only accepts membership fees or same day payments for services.

 

How do I make an appointment?
For your convenience, appointments can be made by clicking above tab for making an appointment, texting, emailing or by calling us.

 

What is the cost of an office visit or membership?
Depending on your level of membership, fees are $100 or $200/month; $300 or $600 per quarter; $575 or $1150 biannually; or $1150 or $2300, which includes a prepayment discount. Non-member visits start at $200 per visit.

 

What is Covered?

You’ll get a yearly thorough physical with age-appropriate screenings (full body skin exam, breast exam, pelvic exam, prostate exam, etc).

 

All office visits (chronic disease management and sick visits, same day or next business day). Communication with your personal physician (email, text, phone, video visits).

 

Many In-office procedures such as skin lesion removal, abscess drainage, ear wax removal, spirometry, nebulizer treatments, cryotherapy, pulse oximetry, vision and hearing screening, EKG, and more at no additional cost.

 

Optional discounted services (low-cost labs, imaging, and medications). You can still use your insurance for labs, imaging, and medications at any facility you choose.

Does anything cost extra?

Dr. Choksy tries to be all inclusive but there are some low-cost extras at times. 

 

If you have a pap smear or a skin biopsy, Dr. Choksy doesn’t charge for the procedure, but there will be a charge for the pathologist to look at the samples under a microscope and give a report.

 

Labs are at wholesale cost and imaging at a very low cost through a local imaging facility.

 

Do you accept insurance?
No! We want to create, build and maintain long-lasting relationships with our patients; therefore, we only work for our patients and do not contract with any health insurance plans to cover office services

 

Is this an insurance plan?

Absolutely not. However, membership can give you a peace of mind that your doctor is available when you need him.

 

It also allows you to know that you’ll always be able to access your personal physician, not an unknown doctor on call.

 

With membership, we will see you the same day or the next business day, so you won’t have to wait weeks to be seen or end up in an urgent care.

 

Do I have to have insurance?

Dr. Choksy will see you as a patient even if you don’t have insurance. The membership fee covers all of your visits and insurance is not billed.

 

You will need to have a health insurance policy that covers you for catastrophic events and specialist visits. 

 

When you become a member in direct primary care, you will receive anti-aging, personalized, high-quality care from a holistic primary care physician. 

 

Can I still be seen there if I have insurance?

Yes! We strongly encourage our patients to have catastrophic coverage for any unplanned events such as hospitalization. For patients with high deductible insurance plans and health savings accounts, we provide an itemized receipt to submit towards their deductible or for reimbursement.

 

Some insurance plans cover fees paid under a DPC membership.  We encourage you to talk to your insurance plan administrator or contact your insurance company for more details.

 

Why would I want to pay a monthly fee if I don't see the doctor every month? Especially when I am healthy.

Most people do not see the doctor every month when you are healthy, but having a personal anti-aging and integrative medicine physician allows you to have that peace of mind of having access to your doctor when you need it the most. It also helps us focus on preventing or reversing chronic diseases.

 

Many of us don't remember when we had a full in-depth exam for prevention of diseases. You will have a full in-depth exam that is different than any other you have had in the past.  We will have our full focus on understanding you from all different aspects. We can help with yoga, nutritional medicine, metabolic medicine, and anti-aging medicine at a personalized level.  By becoming a member, you’ll know that you have a doctor who is ready to answer your questions from where ever you are or see you in the office when you need it the most.  Members also have the benefit of leaving the office with discounted generic medications, which saves your time and money. 

We will do our best so that there is no wait time and same-day/next-day visits for you!

 

If you are traveling internationally, we can use telemedicine and get you seen from where ever you are.  You don't need to worry about finding health care in an unknown city or country.  We can help you through telemedicine and answer your questions. 

 

Think of the monthly membership fee as a way to help cap and lower your medical expenses while improving your health and reversing chronic diseases.

 

Who do I call for medical questions or medication refills?
Membership includes access to a mobile app, which will allow you to securely reach us anytime with any questions. Otherwise, you can call during business hours, or text anytime.

 

What if I need a referral to a specialist?
We will ensure referrals to specialists who provide quality care and will work closely with those specialists to help you achieve your goals.  When possible, we will work to negotiate discounted rates for patients who choose to self-pay; otherwise, you are free to use your insurance for specialist care.

 

How do I obtain laboratory and/or radiology studies?
When I order these studies, charges for them can be covered at the time of your visit or billed to your health insurance directly by our preferred vendors. We have partnered with local labs and radiology offices to provide significantly discounted prices for patients who choose to self-pay.

 

Can my whole family see Dr. Choksy?

Yes, As a family medicine doctor, Dr. Choksy is trained in taking care of all age from pregnancy to end of life age.

 

What happens if I am out of town and get sick or hurt?

We have multiple ways of communication, we can help you while you are out of town with either phone calls, email, text, or video conferencing visits. There is also a network of Direct Primary Care physicians in other areas of the country whom Dr. Choksy can contact and try to get you in to be seen if your injury or illness requires it.

 

How are you able to provide longer office visits and shorter waiting room times? 
We are able to provide this service because we provide care to a limited number of patients per physician. A regular practice will have three to four times patients per provider.  Lower doctor to patient ratios improves overcrowding and overbooking of patient schedules and makes for a more efficient practice.

 

How do you charge lower prices on laboratory services and in-house medications?
We purchase laboratory services and medications from providers at deeply discounted prices and pass those savings on to our patients.

 

What if I need drugs or surgery to stay healthy?
There are effective uses of drugs and surgeries. You can be sure that all the current medical therapies will be available to you if the situation calls for it including pharmaceuticals and specialist referrals. You will be afforded every level of medical care that is needed for you.

 

Can my membership be terminated prior to end of first year?

Yes, it can be terminated if Dr Choksy feels he is not able to serve you well with his services.   If can be terminated if the patient violates the "no call no show"  policy. 

 

What is our policy on missed appointments?

We have a "no call no show" policy which states that patient must call or text to cancel the appointment prior to missed appointment. If the patient missed an appointment and they did not call prior to coming in, then it will be considered that they have violated "no call no show" policy. 

 

Is Dr. Choksy accepting patients?

Yes. Dr. Choksy wants to be able to provide impeccable service to all of his patients and get them in when they need to see him or talk to him. Thus, he will only accept very limited number of patients.
